Bayview Beacon Center Student Enrollment Packet

The Bayview Beacon Center (henceforth denoting all of its subsidiary services, including the ExCel Out of School Time Program) is a program of the Bayview YMCA, and is offered at Phillip & Sala Burton High School. It is open to ALL high school-age youth and adults. The Beacon provides free programs and services such as: homework help, enrichment activities, sports, wellness workshops/case management and employment trainings. Services and activities take place in a safe and supportive environment supervised by teachers and staff.  The Beacon’s mission is to offer tools that will empower participants to live healthy, happy and productive lives.
By completing and signing the following forms, you’re giving permission to become a member of the Bayview Beacon at Burton High School. Beacon members are entitled to take advantage of all Beacon-sponsored services and programs.  Forms should be returned to the Teen Center (room 315) or the Beacon staff office (223).

Bayview Beacon Center Paquete de Matriculación Para Jovenes

El Centro de Bayview Beacon (de ahora en adelante denota todos sus servicios subsidiarios, inclusive el ExCel Out of School Time Program) es un programa del Bayview YMCA, y es ofrecido en la escuela de Phillip & Sala Burton High School.  Está abierto a TODA juventud y adultos. El Beacon proporciona programas y servicios libres como: los deberes, actividades  de enriquecimiento,  deportes, talleres de bienestar / embale gestión y instrucciones de empleo. Los servicios y las actividades suceden en un ambiente seguro y sostenedor.  La mission del Beacon es ofrecer herramientas que autorizarán a participantes para vivir vidas sanas, felices y productivas.  Completando y firmando las formas siguientes, da permiso para que su niño llegue a ser un miembro del Bayview Beacon en la escuela de Burton High School. Los miembros del Beacon tienen derecho para aprovecharse de todos los servicios y programas.  Las formas deben ser regresadas a la oficina del personal del Beacon en el cuarto 223 o  en el cuarto 315 o enviado a: Bayview Beacon, 400 Mansell Street, Room 223, SF, CA 94134.
